1) Peri-implantitis (infection around the implant) - Bone loss around implant leading to loosening caused by plaque bacteria, poor hygiene, untreated inflammation
2) Failure of osseointegration - Implant never properly fuses to bone due to poor bone quality, excessive movement, surgical or healing issues
3) Poor oral hygiene - Inflammation and bacterial buildup caused by inadequate brushing, flossing, maintenance visits
4) Smoking and tobacco use - Delayed healing and increased bone loss causing reduced blood supply and impaired healing
5) Uncontrolled systemic disease - Healing problems and higher infection risk often caused by uncontrolled diabetes and certain immune disorders
6) Excessive biting forces (occlusal overload) - Screw loosening, bone loss, implant and/or abutment fracture caused by bruxism, poor bite design
7) Insufficient bone quantity or quality - Implant lacks long-term support causing bone resorption, osteoporosis, inadequate grafting
8) Surgical complications or improper implant placement - Nerve injury, sinus involvement, instability caused by poor positioning, overheating bone, inadequate planning
9) Mechanical complications - Loose or broken screws and abutments, caused by wear, overload, component compatibility and restorative design issues, improper torque
10) Poor maintenance and lack of follow-up - Small problems progress unnoticed often due to missed recall appointments and delayed intervention
